Choose How You’d Like to Enroll

We are here to help you complete this process of enrolling with Samaritan Advantage Health Plan HMO. Once you have decided which plan you’d like to enroll in, review our pre-enrollment checklist and complete your enrollment any of the following ways:

  1. Enroll over the phone.
    Just call 800-832-4580 (TTY 800-735-2900), Mon.–Fri, 8:30 a.m. to 5 p.m. One of our enrollment experts will assist you in completing your enrollment.
  2. Enroll by mail, fax or drop-off.
    Simply complete our 2021 enrollment form and choose what’s convenient for you. You can mail your form to Samaritan Health Plans HMO, Attn: Enrollment Dept., PO Box M, Corvallis, OR 97339.

    You may fax your form to 541-768-9778, Attn: Enrollment Dept.

    Or you can drop your form in the drop box at the main entrance of our offices at 2300 NW Walnut Blvd., Corvallis. Our office remains closed for Covid-19 precautions, but forms in the drop box are retrieved every business day. Refer to our enrollment instructions for assistance.

Other Options

Medicare beneficiaries may also enroll in a Samaritan Advantage Health Plan HMO through the CMS Medicare Online Enrollment Center located at www.medicare.gov.
